LTA investigation into North-South Line disruptions begun

the following is a media release by the Land Transport Authority:

The Prime Minister has announced that a Committee of Inquiry (COI) will be set up to determine the causes behind the recent disruptions to train services on the North-South Line on 15 and 17 December 2011, which caused inconvenience and distress to commuters. LTA has also separately commenced its investigations into the two service disruptions, even as it works with SMRT to restore the operations of the North-South East-West Line (NSEWL) back to normal.
As regulator, LTA is responsible for investigating all incidents of major service disruptions. As part of its investigations, LTA requires SMRT to provide all maintenance records, operations and system logs, as well as information on its emergency response and incident management, so as to understand the likely cause(s) and contributing factors to the disruptions and the implementation of its contingency plans, and for improvements to be made.
LTA will provide its investigation findings and render its fullest cooperation to the COI.

    What about investigation into possible malpractices, embezzlements and corruptions?
    Surprise not if there is something like the classic SLA and MHA cases or Watevergate scandals may surface after discreet probing by CPIB N CAD.

    i agree that possible malpractices should not be ruled out.
    we can see from history that dictators usually want to hear only good things and praises and would ‘bribe’ their cronies to win their loyalty. This always end up having the cronies abusing the given authority to secretly enrich themselves.

  7. LTA is supposed to be monitoring the SMRT in the first instance;as such,hwo could LTA shoot its own feet?
    it is like MAS was supposed to monitor SIMEX and when that brtish bloke bankrupted BARINGS,an independent audit needed to be carried out.
    to be fair, the gahmen can engage an independent international body to investigate,right?

  10. Any 3rd party involvement? We want full accountablity and transparency from the findings. Nobody must try to hide anything from the public. If the CEO has done a grave mistakes during her tenure, it should be fully disclosed without fear and fervor. Everyone who travels by MRT has a stake to know the truth behind all these lapses.

    services to Sprorean to give them better pay then for Sporean.

  15. Had LTA performed and fulfilled it’s duty as a regulator, such disastrous incidents would not have occurred. Now LHL is trying to convince people LTA will be doing a “thorough investigation”?! They are only doing checks on SMRT maintenance records now?? What had LTA been doing all this time. Count themselves lucky that no serious casualties were taken from the breakdown! Damn! The whole system is nothing but a big coverup!! All trust is broken and there’s ZERO confidence! The 60% should regret and repent! The 40% is suffering because of your actions…

    This process of investigation is too slow! They should already have started it since the first breakdown! The fact that investigation have only begin after a series of problems shows inefficiency in both the government and the LTA! By now, they should have already came out with the solutions rather than investigating.

  21. LTA investigation begun: What is your terms of reference?
    As this breakdown affected 156,0000 public and SMRT is a monopoly providing the only train transportation to the public I think it is only appropriate that LTA announces its terms of reference for the investigation committee. This way LTA will be able to assure the public that it will be transparent. We will also want to know if the terms of reference is aligned with that of SMRT’s own internal investigation and ultimately whether both investigations are congruent with the PM’s soon to be formed COI.
    The public wants to know not only because it concerns all citizens but also so that we can better represent Singapore to the people from other parts of the world we come into contact with. Another important reason is that ordinary citizens can no trust blindly our government’s leadership to get things done right.
    Ronald Reagan once said “trust but verify”, I think many Singaporeans now say “verify before we trust”.

    Twist , they have still FAILED.
    But saw must not resign. We need her to continue till at least 2016.
    The people have always forgotten lessons learnt. They need to be reminded uptill 2016.
    40% must continue to expand to 51%.
    With more FAILURES, BOOBOOS and FIASCOS. technically possible in future, there is hope that even the most APATHETIC may finally find his conscience and wakeup the dragon in them.

    This Ong is not an engineer. He will probably end up declaring the same thing.

    at EOD, report will simply sslaughter some minor scapegoats while shielding the bigger culprits and covering the real problem – MANAGEMENT HEAD and the selection criteria of how MIW and it’s stat boards select ppl. Is it based on connections or is it based on MERITOCRACY which PAP has always been championing?????

    you’re absolutely right! even in any small suspected commercial crime case, our police and the cad would just carry out raids and not wait for the suspect to provide records. this is really a load of bs!
    and about iso, this is truly another big big scam. i was told by several friends from corporates that the iso appointed auditors would always call them several weeks in advance to get their records prepared for audit. and when they failed in the first audit, they will be given time again to adjust their records for a second audit, until they passed.
    i was further told that if the auditors are too stringent and go strictly by the books, their company will lose the business from the iso certified company, who will appoint another auditor.
    moreover, both iso auditors and iso certified companies are fully aware themselves that if the companies were to really apply wholesale the iso standards set, they would end up doubling the number of employees to do all the recording, and employees from different departments would each end up protecting their own turfs – especially between the quality control and production. the end result is the company suffers.
    what a contradiction! so, the net result is: the iso auditor would provide tips and hints to the company for their own pre-audit meetings to instruct each department on what record to provide truthfully and what could be doctored.
    so, don’t be duped and don’t think so highly of all the iso certifictions. and if the iso certified corporations are govt owned, related to glc’s and the like, can you trust that their iso certs are more authentic? i think you know the answer.

  29. With regards to the handling of breakdown or emergency, there is blatant negligence. What is there to investigate? People had to smash window for air, were left in the dark (literally & metaphorically) and had to walk on the tracks.
    It is clear either there was no protocol or they did not follow it. LTA is the one who must know if there is a detailed procedure for handling breakdowns. If there was no such procedure and LTA did not demand for it & they approved SMRT’s operation, then LTA is answerable and ought to be fired too. The same if there was a procedure but not workable and LTA approved it.
    If SMRT did have the procedure for breakdown submitted and approved but did not follow it, then LTA should go after them. What is there to investigate as far as emergency handling is concerned?
    Note: For me to be banned from ST forum, I must be darn close to uncovering something. All the signs show one thing – they have been playing dirty and still is and will be.

    U people suspect whether the investigation will be clean or not.
    The fact is 60% of citizens purposely gave them such an extent that they control basically everything. Now you all go ask your papa n mama what have they done to you.
    Their ppl are placed in all the many juicy positions. Ppl grow up with value system that to succeed you should join them. This 50 year brain training created a unique society of ppl.
    You can only see change when all existing old batch go for tea with hussein

    Lets be fair to SMRT and LTA.
    We all know SMRT is Government Related.
    We all know how much of singapore is controlled by the government.
    We all know what we all already know.
    If we really know what is the ROOT CAUSE of all unhappiness , then we should know that its the 60% who voted for them.
    The 40% can only accept the decisions of the 60% . I know it could be painful and bitter to the 40% but this is what Democracy is all about. Its a very simple and ancient concept and most importantly very basic and simplistic.
    The winner wins and the loser loses.
    The country goes in the direction of the 60%.
    This 60% voted in support of influx policy by their mandate.
    The 35% voted for tony tan to be the president regardless of him being a chairman (dy) of the GIC. so he can check on GIC being an ex-chairman (dy).
    It may be funny to the 40% who may question on his suitability and independentness to carry out check on his past.
    But remember, Democracy.
    A very overly simplistic concept. majority, even by a bit, is declared the winner. As low as 35%.
    We the people have Accepted this system for over 50 years in a row.
    The only way to change is by the People. Only the People can change singapore.
    So, we can see clearly, its the People’s work and decision that resulted in today’s problems we facing.
    Majority are not the wisest in everything. This is one of reasons why the top are the minority. A pyramidal hierarchy.
    Yet, democracy is based precisely on Majority.

  32. Can someone explain to me why is there a need to have COI, SMRT and LTA to be doing the investigation? SMRT is at fault here. LTA has to investigate and share with public and the government their findings. I bet the objectives are all the same right? What caused it, what needs to be done to rectify it, what are the recommendations that need to be implemented, the timelines and in the event such situations happen again, what steps need to be taken? It would be great if the objectives for each body that is investigating the disruption be made public NOW. There are costs to all these. Make sure these costs are not trasferred to commuters!
